Primary
Responsibilities:
* Assessing geriatric cases, ordering lab results and prescribing
medication
* Managing primary and episodic care for the Evercare enrollees
* Establish and maintain effective relationships with Evercare enrollees,
key decision makers, facilities and collaborating physicians
* Performing physical examinations and working with patients' primary
care physicians
* Interfacing with families at regularly scheduled intervals
* Interfacing with the nursing staff, doing clinical in-services,
and collaborating with unit managers
* Documentation in an Electronic Medical Record
