Basic --
1) 18 years of age or older;
2) Current registration from the National Registry of Emergency Medical
Technicians (NREMT);
3) Certificate of successfully completing an Arizona EMT-B training
course approved by the Department of Health Services; or certificate
of an Arizona EMT-B refresher course if applicant has current certification,
licensure, NREMT registration, or NREMT re-registration eligibility
at the basic emergency medical technician level or higher level
Intermediate --
1) 18 years of age or older;
2) Evidence of current NREMT-Intermediate registration; (score of
at least 70% on exam);
3) Certificate of successfully completing an Arizona EMT-I training
course approved by the Department of Health Services; or certificate
of an Arizona EMT-I refresher course if applicant has current certification,
licensure, NREMT registration, or NREMT re-registration eligibility
at the intermediate emergency medical technician level or higher level;
Paramedic --
1) 18 years of age or older;
2) Evidence of current NREMT-Paramedic registration (score of at least
70% on exam);
3) Certificate of successfully completing an Arizona EMT-P training
course approved by the Department of Health Services; or certificate
of an Arizona EMT-P refresher course if applicant has current certification,
licensure, NREMT registration, or NREMT re-registration eligibility
at the paramedic emergency medical technician level;
By Reciprocity: Partial (see below)
If applicant has current EMT certification or licensure in another
state or jurisdiction, current NREMT registration, or current NREMT
re-registration eligibility, applicant can apply for initial two-year
Arizona EMT certification after completing an Arizona ALS refresher
or EMT-B refresher course rather than an Arizona EMT-B, EMT-I(99),
or EMT-P course.
Certification Fee: Initial - None; Renewal
Continuing Education: 60 clock hours every two years
